Abstract

Obesity in adolescence is a serious global health challenge due to the long-term risk of degenerative diseases and decreased psychosocial quality of life. This study aims to describe the implementation of family-based community midwifery care in adolescents with obesity to increase independence in healthy lifestyle management. This case study uses a comprehensive midwifery care approach through Varney's steps, including assessment, problem identification, planning, implementation, and evaluation. The subject of the study was Ms. Z (16 years old) in Dusun II, Bangun Rejo Village, who was obese with a Body Mass Index (BMI) of 33.45 kg/m² and chief complaints of fatigue and frequent sweating. The intervention provided focused on education on balanced diet management, increasing daily physical activity, and regulating adequate rest patterns. During the mentoring period from February 12 to May 2, 2026, regular monitoring of client behavior changes was conducted. The evaluation results showed significant behavioral improvements, where the client became more disciplined in managing nutritional intake, exercising regularly, and reported a fitter and lighter body condition compared to before the intervention. The success of this care program confirms that a structured, educational, and persuasive community midwifery approach is highly effective in increasing adolescent awareness and independence in weight management. It is hoped that the results of this care program can serve as a reference for health workers in optimizing their promotive and preventive roles in adolescents at the community level to prevent future complications of degenerative diseases, while also emphasizing the importance of family support as a determining factor in the success of sustainable healthy lifestyle modifications in adolescents.
