CASE STUDY MIDWIFERY MANAGEMENT FOR POSTPARTUM BREAST MILK CONTAINMENT AT PMB RIMENDA

Background: Based on the third Sustainable Development Goal (SDGs) target and the World Health Organization (WHO) recommendation, exclusive breastfeeding for the first six months is an important intervention to reduce infant mortality, but its success is often hampered by lactation problems such as breast engorgement experienced by approximately 31% of postpartum mothers. Breast engorgement due to suboptimal emptying causes pain, hard breasts, and obstructed milk flow which if not treated immediately can trigger mastitis complications. Objective: This case study aims to implement comprehensive midwifery care management for postpartum mothers with breast involvement at PMB Rimenda Br Tarigan in 2026. Method: The research method used is a descriptive case study with the Helen Varney 7-Step Midwifery Management approach on Mrs. S, 34 years old, P3A0, fifth day postpartum. Data were collected through assessment, physical lactation examination (inspection and palpation), and clinical observation. Results: The assessment showed that the patient complained of right breast engorgement, tension, hardness, tenderness, and low milk flow. Non-pharmacological management included breast care, lactation massage stimulation, education on proper breastfeeding attachment techniques, and the application of a combination of warm and cold compresses. Final evaluation showed significant improvement, with softer breast texture, better milk flow, reduced pain, and resolved maternal anxiety. Conclusion: Implementation of evidence-based midwifery care effectively addresses breast involvement, prevents serious complications, and supports successful lactation in postpartum mothers.
