Abstract

Continuity of Care (CoC) is a comprehensive and continuous midwifery service provided to women during pregnancy, childbirth,postpartum, newborn care, and family planning. The purpose of this study was to provide comprehensive midwifery care to a pregnant woman from the third trimester through postpartum, newborn care, and family planning services using a continuity of careapproach. This study used a case study method conducted on one pregnant woman with a gestational age of weeks. Data were collected through interviews, physical examinations, observations, and documentation. The midwifery care was carried outaccording to the standards of midwifery practice and documented using the SOAP method. The results showed that the pregnancy progressed normally without complications. The labor process was spontaneous and normal, resulting in the birth of a healthy baby. The postpartum period and newborn care were conducted appropriately, and both mother and baby remained in good condition. Family planning counseling was also provided, and the mother chose a suitable contraceptive method according to her needs. Inconclusion, continuity of care midwifery services can help monitor maternal and neonatal health conditions comprehensively, detectcomplications early, and improve the quality of maternal and child health services.
